The Opportunity Seeking Family Care or Primary Care physician assistants and nurse practitioners. Current national certification, DEA, and BLS are required. At least one (1) year of primary care experience is required. Fingerprinting via DOJ will be required. Current CA state license is required. The Practice San Bernardino Health Center – San Bernardino, California Adelanto Community Health Center – Adelanto, California Ontario Health Center – Ontario, California A Federally Qualified Health Center. Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker. Facility hours for this location are Mon-Fri 8am-5pm. Good mix of patients including peds. 15-18 patients per day per provider. Point of Care testing available at site; minimal labs.
