Job Description Primary Care Physician – Senior-Focused
Outpatient Practice San Antonio, TX Salary: $225K-315K A rapidly
expanding, senior-focused outpatient care organization is seeking
dedicated Primary Care Physicians who want to practice medicine
with purpose. This model is built for physicians who value time
with patients, strong team-based support, and a meaningful role in
improving the health and well-being of older adults. This is a
full-time Monday–Friday position (8am–5pm) with no nights, no
weekends, and no inpatient responsibilities. Why This Role Stands

What You’ll Do Provide compassionate, comprehensive primary care to
a senior population in an outpatient-only setting. Evaluate and
treat patients using evidence-based standards. Work closely with a
multidisciplinary care team—including NPs/PAs, nurses, behavioral
health specialists, social workers, clinical pharmacists, care
coordinators, and quality-based coders. Participate in daily
huddles, collaborate on care plans, and ensure accurate
documentation aligned with quality and value-based standards. Act
as a leader within the center by promoting coordinated care,
proactive outreach, and exceptional patient experiences. What Makes

What You Bring MD or DO with board certification or eligibility in
Family Medicine, Internal Medicine, or Geriatric Medicine. Current
or attainable medical license in the state of practice. Strong
interest in senior care and value-based medicine. Excellent
communication and interpersonal skills. Experience in geriatrics,
Medicare Advantage, VBC, HEDIS, or EMR systems is helpful—but not
required for motivated learners. Candidates must be pursuing
full-time, Monday–Friday employment. Compensation & Benefits
Competitive base salary range of $225,000–$315,000 depending on
experience and location. Robust bonus opportunities tied to quality
