This on-site role involves evaluating patients’ physical conditions, developing and implementing personalized treatment plans, and providing high-quality physical therapy services directly within patients’ homes. You will closely collaborate with healthcare teams, educate patients and their families on post-therapy care, and maintain accurate documentation of treatments and progress. Qualifications Proficiency in Physical Therapy techniques, Home Care protocols, and Rehabilitation strategies Extensive knowledge in Medicine and Patient Care principles Strong communication and interpersonal skills for working with patients and families Current licensure as a Physical Therapist in California Reliable transportation for travel to patient locations Experience in home care settings and a thorough understanding of related regulations is a plus
