As an RN Case Manager in Home Health, you will play a crucial role in managing patient care plans, ensuring the delivery of excellent medical, nursing, and supportive care in the comfort of our clients' homes. This position requires a combination of clinical expertise, compassionate communication, and detailed coordination across multiple disciplines to promote optimal health outcomes. Assess patient conditions during visits and coordinate with healthcare teams to modify care plans as needed. Educate patients and families on disease processes, management, and prevention strategies, executing tailored in-home health interventions. Supervise and train junior nurses and other healthcare staff, maintaining a strong, supportive team environment. Coordinate resources and services between community organizations and families to support patient care and recovery. Maintain timely and accurate patient records, complying with all legal, regulatory, and accrediting requirements. Strong clinical skills in a home health setting Excellent organizational and communication skills Ability to handle complex case management Proficient in using healthcare databases and patient management software Medical Insurance Dental Insurance Vision Insurance 401k Valid and current Registered Nurse (RN) license in the state of practice Certification in Case Management (CCM) preferred but not mandatory
