Job Description:
A company is looking for a Care Manager (RN) (State).
Key Responsibilities
Develops and facilitates personalized care plans for members with
complex physical needs
Coordinates care management interventions and collaborates with
providers to address members' unmet needs
Monitors member status and revises care plans as necessary to meet
goals and needs
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in California
